7-Ketocholesterol (7-KC) is a major oxysterol formed through the autoxidation of cholesterol, particularly under conditions of oxidative stress. As a stable and abundant product of lipid peroxidation, 7-KC serves as a key biomarker of lipoprotein cholesterol oxidation and is increasingly recognized for its pathological role in neurodegenerative diseases. Unlike native cholesterol, 7-KC exhibits potent cytotoxic, pro-inflammatory, and pro-oxidant properties. It disrupts membrane integrity, impairs mitochondrial function, and induces oxidative stress, leading to neuronal apoptosis and glial activation. These effects are mediated through multiple mechanisms, including the generation of reactive oxygen species (ROS), activation of inflammatory signaling pathways, and interference with cholesterol homeostasis. In the context of neuroscience, elevated levels of 7-KC have been detected in the brains and cerebrospinal fluid of individuals with Alzheimer's disease, Parkinson's disease, and multiple sclerosis. Its accumulation contributes to synaptic dysfunction, myelin degradation, and chronic neuroinflammation-hallmarks of progressive neurodegeneration. Due to its stability and bioactivity, 7-KC is not only a valuable biomarker for oxidative damage but also a promising target for therapeutic intervention.
