- Erythropoietin (EPO) is a glycoprotein cytokine primarily produced by the kidney and liver in response to hypoxia. This hypoxic signal is mediated by the HIF transcription factor, whose stability is regulated by oxygen-sensing prolyl hydroxylases. EPO's principal function is to regulate erythropoiesis by promoting the survival, proliferation, and differentiation of erythroid progenitor cells. It exerts its effects by binding to its specific receptor (EPOR). The canonical high-affinity receptor is a pre-formed EPOR homodimer whose activation triggers the JAK2/STAT signaling pathway. An alternative heteromeric receptor complex, incorporating the beta-common subunit, has been proposed to mediate tissue-protective effects in non-hematopoietic cells, such as those in the neuronal, cardiovascular, and renal systems, though its expression and function remain an area of study. Beyond its critical role in red blood cell production, EPO exhibits pleiotropic functions, including tissue protection under ischemic stress and promotion of angiogenesis. Clinically, recombinant human EPO is a vital therapeutic agent for treating anemias, particularly in chronic kidney disease and during chemotherapy.
