Tau 441 (2N4R) is the longest human brain tau isoform, containing two N-terminal inserts and four microtubule-binding repeats. It stabilizes neuronal microtubules, but its pathological hyperphosphorylation leads to paired helical filament aggregation. Tau 441 constitutes the core component of neurofibrillary tangles in Alzheimer's disease and related tauopathies.
