- High-density lipoprotein (HDL) is a crucial plasma lipoprotein, present at concentrations of 1.0-2.0 g/L in healthy fasting individuals. HDL plays a central role in reverse cholesterol transport, a process in which it collects excess cholesterol from peripheral tissues and transports it back to the liver for excretion or recycling. This function helps maintain cellular cholesterol balance and prevents the buildup of cholesterol in blood vessel walls. HDL also facilitates cholesterol esterification, converting free cholesterol into cholesteryl esters for safe transport in the bloodstream. Beyond lipid transport, HDL exhibits anti-inflammatory, antioxidant, and antithrombotic properties, contributing to vascular health. Clinically, higher HDL levels are strongly associated with a reduced risk of coronary heart disease and atherosclerosis, making it a key biomarker in cardiovascular risk assessment. Therapeutic strategies aimed at raising HDL or enhancing its function are under investigation, with the goal of reducing cardiovascular events and improving long-term heart health.
