- Platelet factor 4 (PF4), also known as CXCL4, is a 70-amino acid chemokine stored in the alpha granules of platelets and released during platelet activation and aggregation. PF4's primary function is to bind with high affinity to heparin and other heparin-like molecules on the endothelial surface, neutralizing their anticoagulant effects and thereby promoting blood coagulation. Beyond heparin neutralization, PF4 acts as a chemoattractant for neutrophils, monocytes, and fibroblasts, contributing to inflammation and wound repair. PF4 also directly activates platelets by binding the thrombopoietin receptor (c-Mpl), triggering the JAK2-STAT signaling pathway and promoting platelet aggregation. Clinically, PF4 is central to the pathogenesis of heparin-induced thrombocytopenia (HIT), a serious immune-mediated adverse reaction where antibodies form against PF4-heparin complexes, causing platelet activation, thrombocytopenia, and thrombosis. Similar immune responses underlie vaccine-induced immune thrombotic thrombocytopenia (VITT), seen rarely after certain COVID-19 vaccinations. PF4 measurement and antibody detection are important in diagnosing these conditions. Additionally, PF4 has antimicrobial properties, such as killing malaria parasites within red blood cells.
