- Tetrahydrobiopterin (BH4) is an important coenzyme involved in various biochemical processes, including serotonin, dopamine, norepinephrine, adrenaline, and melatonin. It has the function of antioxidant and scavenging active nitrogen oxides in human body, and plays a key role in diabetes, pulmonary hypertension, pathological myocardial remodeling and other diseases. In addition, BH4 has been found to play a dual role in promoting and inhibiting the occurrence and development of malignant tumors. Tetrahydrobiopterin deficiency is a rare autosomal inherited disease, which belongs to hyperphenylalaninemia (HPA) along with phenylketonuria. Its characteristic is insufficient synthesis of monoamine neurotransmitters dopamine and serotonin due to BH4 biosynthesis or circulatory disorders. This disease can be better controlled by delaying diagnosis. Meanwhile, studies have shown that BH4 induces resistance to proteasome inhibitors and tumor progression in multiple myeloma, and its key role in radiosensitivity regulation has also been confirmed.
